SCHOOL OF HEALTH INFORMATION MANAGEMENT
2018/2019 ND AND HND ADMISSION



Applications are invited from successful candidates who scored 150 and above in 2018 UTME (ND applicants) for admission into the School of Health Information Management of the University College Hospital, Ibadan for a Two-Year Programme leading to the award of the National Diploma (ND) and also a Two-Year Higher National Diploma (HND) Programme in Health Information Management.  The course, which is non-residential, shall commence in October 2018

1. ENTRY QUALIFICATION FOR ND PROGRAMME
Apart from the JAMB result, applicants must have obtained the minimum entry requirement of (5) credits at not more than two sittings in SSCE/GCE/WASC/NECO in the following subjects:

  1. English Language
  2. Mathematics
  3. Biology
  4. Physics or Chemistry
  5. Any other subject

2.  CONDITIONS FOR ADMISSION (HND PROGRAMME)

Selection of candidates into National Diploma & Higher National Diploma will be on the basis of good academic qualification, good performance in the Computer Base Test (CBT), and oral interview as well as presentation of relevant ORIGINAL CERTIFICATES.
MERIT AND PASS IN (NATIONAL DIPLOMA) ND SHOULD NOT APPLY EXCEPT WITH ND PRACTICING LICENSE OR 3 YEARS WORKING EXPERIENCE
The Computer Base Test (CBT) for ND will be in:

  1. English Language
  2. Mathematics
  3. General knowledge
The Computer Base Test (CBT) for HND will be in:
  1. English Language
  2. Health Records Management
  3. Anatomy & Physiology

Please note that:

i. pregnancy is not allowed during training
ii. Selected candidates will be required to undergo and pass medical examination, first thing after admission


3.  METHOD OF APPLICATION

Interested ND applicants are to pay a non-refundable fee of N2, 000for screening while HND applicants are to pay N7, 500 for Application and Screening fee respectively into the TSA ACCOUNT (TSA/UCH/SHIM/ADMISSION FORM) in any commercial bank.  Applicants are expected to convert their bank teller into a receipt at the Cash Office of the Finance & Accounts Department, University College Hospital, Ibadan.  The photocopy of the receipt of payment and the photocopy of the bank teller should be forwarded/brought to the Administrative Office of the School.
All applicants must have a correct and functional e-mail address and are expected to collect their Access Code from the school Administrative Office to apply online, that is, to Log onto: www.shim.uch-ibadan.org.ng  Application must be completed online, not later than Monday, 27th August, 2018.
The Computer Base Test (CBT) for the ND & HND programme will be conducted at the Smart Class two (2) Second Floor University College Hospital, Ibadan on a later date and time which will be communicated to all applicants through text messages and e-mail.
Successful applicants will be invited for an oral interview at a later date through text messages and the School’s website.

UCH is making life hard for us as patients. we pay thousands here in this hospital so is it going to be okay or nice enough to start paying for parking space too. this another means of privatization that we are shouting we don't want. this was said by a patient glancing through the fee when the press crew was moving around the venue. Another patient said, only places where people catch fun is where gate fee or park spaces are paid for. But imagine we have a family member here lying down, we are yet to gather the funds for that, so it is car parking tariff that we should be looking for now  .
